How an NCLEX case study works
This is the format that carries the clinical judgment measurement, and the one that punishes treating each item as a fresh start.
An NCLEX case study is a set of six items that share the same client presentation, with the clinical picture unfolding between them. Clinical judgment is measured by 18 such items — three sets of six — plus approximately 10% stand-alone items. Because the items share one patient, what you established earlier still applies: the case is a single reasoning chain, not six questions that happen to be adjacent.
What makes a case different from six questions
The information unfolds. A vital sign that was normal at the first item may not be at the fourth, and the reason the case is worth working in order is that the change is the point. Answering item 5 without carrying item 2 forward is the specific failure this format is built to expose.
- The items map onto the six stepsA case typically walks from noticing findings through to evaluating the response, which is why the step tags on your wrong answers cluster meaningfully within a case.
- The formats vary within one caseA matrix, then a bow-tie, then an ordering item — against the same patient. Meeting a format for the first time inside a case is a bad way to meet it.
- Nothing resets between itemsThe presentation is continuous. A conclusion you reached and abandoned is a conclusion you will be asked about again.
How to work one
- Read the whole record before the first item. The chart is the case; the item is a question about it.
- Write down the priority you settle on. If it changes at item 4, notice that it changed and why.
- Do not shuffle. Working a case out of order turns it back into loose questions and throws away what the format tests.
- Expect the formats to vary. The scoring differs between them — see the item types.
