The Client Needs categories
Grouped as the test plan groups them, because the grouping is what tells you which areas are being weighed against each other.
The NCLEX-RN test plan has four major Client Needs categories. Safe and Effective Care Environment and Physiological Integrity are each divided into subcategories, giving eight categories and subcategories in total. Each carries a published percentage range of the exam, and no category falls below 6–12% — there is none safe to skip.

How to read the weighting
- Physiological Integrity is the biggest group by far. Its four subcategories together span roughly 39–63% of the exam.
- Management of Care is the largest single line at 15–21% — delegation, scope, prioritisation and advocacy.
- The two smallest are still 6–12% each. On a longer exam that is a real number of items.
